Overview.....
The aim of this questionnaire is to create a
personal development tool box of courses and activities that SomerVille
residents have experienced and believe to have been a valuable contribution to
their life journey, and recommend to others. (This tool box will be posted on the SomerVille website
in a new Self Awareness page and will assist SomerVille residents to
develop the behavioural tools required to live life congruent with the
SomerVille values).
The information you provide in this survey will
only be posted on the SomerVille Website if you consent to being contacted
for more information about your experience in this course or activity. As
this is the case you only need to fill out the survey for courses or
activities in which you do consent to being contacted.
Please complete one survey for each course or
activity you have done (i.e. if you have done two
courses then fill out this survey twice, one for each course).
